    This will be our public reporting site to keep our community informed of any residents and staff with confirmed COVID.
    As of this date, 6.25.2020 we have ZERO confirmed and/or suspected cases involving staff or residents.

    On July 6, 2020 we will take part in the mandatory testing per Governor Beshear’s directive. All staff and residents will be tested with results available within 24-48 hours.

    On 7/18/2020 the facility was made aware of one staff member who tested positive for the novel coronavirus also known as COVID-19. This employee last worked on 7/14/2020, and did not have direct contact with residents/patients of the facility. At this time there are no residents or staff that are showing symptoms of the virus and no tests are pending.

    Facility leadership attempted to make contact with Power of attorneys, guardians, and/or responsible parties on 7/18/2020 by phone. At this time visitation, communal dining and activities will be suspended for a minimum of 14 days per state guidance.

    on 8/17/2020 the facility was made aware of a positive result from our bi-weekly testing conducted on 8/14/2020. The employee is a direct care provider who who remains asymptomatic. A review of the employees assignments and contacts within the facility was conducted by center leadership. All employees and residents who were exposed to this individual will be tested weekly for a minimum of 14 days. Remaining staff members who have not been exposed will be tested on a bi-weekly basis per state guidance.

    Facility leadership has attempted to notify all responsible parties including Power of Attorneys and court appointed guardians by phone on 8/17/2020. In effort to mitigate the risk of exposure to remaining staff and residents the center will cease all communal dining programs, group activities and visitations per the state guidance released on June 25, 2020.

    On 8/31/2020 the facility was notified that one employee tested positive during routine bi-weekly testing conducted on 8/27/2020. This information has been provided to the local health department and the employee remains on quarantine per CDC guidelines. In accordance to the newly released CMS guidance ALL residents will be tested weekly until 14 days have passed from the last positive.

    Nursing leadership are in the process of notifying responsible parties, including POA and guardian, by phone. The center will cease communal dining and activity programs per guidance issued on 6/25/2020.

    100% of residents tested on 9/1/2020 were negative for COVID-19 virus. Residents will be tested again within the next 7 days. Staff are scheduled for routine testing today. Updates to follow.

    We’ve received all employees who completed routine testing on 9/3/2020 and are 100% negative. Positivity rate for the county is at 5.8% which requests continued weekly staff testing.

    On 9/28/2020 the facility was notified that three employees tested positive during routine weekly testing conducted on 9/25/2020. This information has been provided to the local health department and the employees remain on quarantine per CDC guidelines.     The facility was made aware of one resident who tested positive on this date. All residents were tested in response to the staff positive on 9.28.2020 and at this time no residents are symptomatic. Additional protective measures have been implemented in response to these results. There are several tests, both staff and residents, that remain pending. As results are received updates will be made to this page.

    At this time all remaining resident tests performed 9/30/20 have resulted and all were negative except the one positive resident mentioned in the previous update. All residents will be tested again on Wednesday 10/6/20 per CMS guidelines.

    Per the Hopkins County Health Department investigation there are Five (5) employees who have been placed on quarantine for potential COVID-19 related symptoms. These employees recently underwent Nasal PCR testing on 10.1.2020 all of which were negative. Updates will be released as new information is provided.

    This date facility was made aware of one employee who has tested positive, last date worked was 9/29/20. All resident’s and staff will be retested weekly until 14 days have passed with no new positive employees or residents. The next resident test date will be 10/7/20 and employee test date will be 10/8/20.

    On 10/9/20/20 the facility was made aware of one (1) employee positive resulting from the weekly COVID-19 testing. This employee is asymptomatic but will quarantine for at least ten (10) days per CDC and KY return to work guidelines. Residents will continue weekly testing until fourteen (14) days have passed since the last known positive.

    Hopkins County has been elevated to a “red” county which means that all staff working in our facility will be required to test twice a week until the county positivity rate is lowered. We have secured testing dates for this upcoming Monday and Friday for all staff. Additional positive cases and/or symptomatic cases of three (3) or more will be reported.

    On October 15, 2020 a COVID-19 Focused Infection Control Survey was initiated at our center. On October 16, 2020 the survey team exited and found no deficient practice noted.

    The center was notified on this date of ONE employee who tested positive for COVID-19. This individual is asymptomatic but will be quarantined per CDC and state guidelines. All residents who were cared for by this individual will be placed on quarantine for the next 14 days and will continue to have routine testing.

    The center was notified on 10.26.2020 that THREE asymptomatic employees tested positive from routine testing held on 10.23.2020. These employees will be placed on quarantine per CDC and state guidelines. All residents who were exposed or had potential exposure will be tested weekly for 14 days.

    On 10/29/2020 our facility was notified that ONE employee tested positive for COVID-19. This employee has been on quarantine since 10/22/2020 due to a direct exposure and later became symptomatic during quarantine. The facility has notified both the employee and Hopkins County Health Department of the positive results. This individual will continue to quarantine per the local health departments guidance.     The center was notified today that one of our PRN staff members has tested positive during routine surveillance testing. At this time the employee is asymptomatic and will quarantine at home per the CDC and commonwealth guidelines. Residents who have had direct contact will be placed on quarantine and have testing every 3-7 days until 14 days have passed. At this time the employee will quarantine at home per the CDC and commonwealth guidelines.     We were notified on 11/16/2020 of ONE employee positive from routine testing on 11/13/2020. This employee last worked at the center on 11/16/2020 and is now quarantined per CDC and state guidance. Residents who were in direct contact with this individual will be quarantined and undergo routine testing weekly until 14 days have passed. I would also like to mention that all residents tested last week were negative! All employees will continue to have routine testing twice weekly until further notice.     This notice is a follow-up from the post dated November 16, 2020. The center was made aware that an additional staff member and one resident have now been confirmed with COVID-19. This brings the total confirmed cases to three for November 16, 2020. At this time there are no symptomatic staff or residents to report. All residents and staff will undergo routine testing on this date. Staff continue to wear surgical grade masks and eye protection (goggles or shield) regardless of the residents status. In addition to these measures all residents with known or potential exposure will have increased surveillance and screening for COVID-19 symptoms.     The center has been made aware this date of 1 resident with confirmed COVID-19, 2 staff and 8 residents with probable Covid-19 based on rapid antigen testing. Suspected residents have been moved to a quarantined unit within the facility. All residents and staff will undergo routine testing again on Tuesday 11/24/20.     Today the facility was made aware of six additional employees and one resident all of which confirmed by a molecular PCR test from routine surveillance. Additionally, the facility identified one new resident as a probable case through rapid COVID-19 antigen testing. These numbers bring facility totals to: eight confirmed staff, and nine confirmed residents. Center leadership attempted to contact POA’s and/or first emergency contact by phone today to discuss ongoing mitigation and infection control plans.     The facility was made of aware of one symptomatic staff member on 11/22/2020 and a rapid COVID-19 antigen test was performed. This employee tested positive for the SARS-CoV-2 virus and was immediately placed on quarantine per CDC guidance. The facility is working with the local health department. Routine surveillance testing for residents and staff will be conducted on 11/24/2020.
