    This will be our public reporting site to keep our community informed of any residents and staff with confirmed COVID.
    As of this date, 6.25.2020 we have ZERO confirmed and/or suspected cases involving staff or residents.

    On July 6, 2020 we will take part in the mandatory testing per Governor Beshear’s directive. All staff and residents will be tested with results available within 24-48 hours.

    Update from mandatory testing 7/6/2020
    All results from staff and resident tests have resulted: 100% Negative.

    On 7/18/2020 the facility was made aware of one staff member who tested positive for the novel coronavirus also known as COVID-19. This employee last worked on 7/14/2020, and did not have direct contact with residents/patients of the facility. At this time there are no residents or staff that are showing symptoms of the virus and no tests are pending.

    Facility leadership attempted to make contact with Power of attorneys, guardians, and/or responsible parties on 7/18/2020 by phone. At this time visitation, communal dining and activities will be suspended for a minimum of 14 days per state guidance.

    on 8/17/2020 the facility was made aware of a positive result from our bi-weekly testing conducted on 8/14/2020. The employee is a direct care provider who who remains asymptomatic. A review of the employees assignments and contacts within the facility was conducted by center leadership. All employees and residents who were exposed to this individual will be tested weekly for a minimum of 14 days. Remaining staff members who have not been exposed will be tested on a bi-weekly basis per state guidance.

    Facility leadership has attempted to notify all responsible parties including Power of Attorneys and court appointed guardians by phone on 8/17/2020. In effort to mitigate the risk of exposure to remaining staff and residents the center will cease all communal dining programs, group activities and visitations per the state guidance released on June 25, 2020.

    On 8/31/2020 the facility was notified that one employee tested positive during routine bi-weekly testing conducted on 8/27/2020. This information has been provided to the local health department and the employee remains on quarantine per CDC guidelines. In accordance to the newly released CMS guidance ALL residents will be tested weekly until 14 days have passed from the last positive.

    Nursing leadership are in the process of notifying responsible parties, including POA and guardian, by phone. The center will cease communal dining and activity programs per guidance issued on 6/25/2020.

    100% of residents tested on 9/1/2020 were negative for COVID-19 virus. Residents will be tested again within the next 7 days. Staff are scheduled for routine testing today. Updates to follow.

    We’ve received all employees who completed routine testing on 9/3/2020 and are 100% negative. Positivity rate for the county is at 5.8% which requests continued weekly staff testing.
